I didn't use a recipe so I'll try to recall as best possible.

2 liter A&W Cream Soda
1 bottle Smuckers Butterscotch syrup
1/2 cup heavy whipping cream

The chilled version (Matt's fav) is the easiest.

In a mug mix 1 3/4 cup cream soda with 1 1/2 - 2 Tbsp butterscotch
syrup, stir well.

In a mixing bowl, whip cream with 2 Tbsp butterscotch. Whisk til
frothy but not fully whipped.

Place a dollop of cream onto of soda and enjoy.

It really is yummy.

For the frozen, I froze the cream soda in ice cube trays for a few
hours, til just slushie like.

 It was realy yummy my sister and i loved it i added 1 extra Tbsp i found it weak before it but awesome after
